ANITA CHAPMAN, AGACNP-BC

Anita Chapman is a board-certified Adult Gerontology Acute Care Nurse Practitioner (AGACNP-BC), specializing in the care of older adults. She is dedicated to providing compassionate, evidence-based care to persons 13 years and older, with a focus to the unique health needs of the geriatric population, using a strong focus on preventative medicine and chronic disease management. She believes in a collaborative, patient-centered approach. She believes in fostering a true partnership with each individual, moving beyond prescriptive directives to co-create personalized wellness plans. These plans are designed to align with each patient's unique lifestyle, values, and health objectives, empowering them to actively participate in and take ownership of their health journey.

She earned a Master of Science in Nursing from Grand Canyon University. Prior to becoming a NP, she spent 22 years as a registered nurse in level 0ne trauma/burn unit and emergency department, giving her a deep understanding of comprehensive patient care.

Outside of her professional commitments, she finds enjoyment and balance through a variety of personal pursuits. She enjoys exploring new destinations through travel, cultivating her garden, defining culinary skills, and most important, cherishing the quality time and creating lasting memories with her family.
